Job Description:
POSITION SUMMARYThe Crisis Therapist is responsible for
completing assessments, determining safety, danger of DTO/DTS, and
the level of care needed by clients presenting in the crisis
department. They welcome and coordinate the determination of care
for all clients in coordination with the medical provider, the
paramedic, or RN.ESSENTIAL RESPONSIBILITIESThe crisis therapist
initiates an assessment (or reviews accompanying assessment if
completed by participating provider) within 30 minutes of the
clients arrival to determine if the individual meets basic
requirements for admission to a Polara Health inpatient facility.
Including:Crisis Screening and assessmentColumbia Suicide Screening
ScaleBroset Violence Checklist (if indicated)Determine the
appropriate level of care using the LOCUS evaluationCompleting
Safety Plan and coordinating appropriate care provisionsCoordinates
sending out packets to find appropriate placement for clients not
admitted or diverted Collaborates with client supports, Nursing
Staff and Provider to develop and implement an individualized
person-centered plan of care to ensure safety and appropriate
therapy interventions Gathers information from the patient and
available parties to provide a thorough assessment of what the
patients immediate needs are for treatment and safety, and
appropriately documents them Evaluates needs and safety concerns of
the client Collaborates with the interdisciplinary treatment team
to assess packets, and determine the appropriateness of referrals
for admission. Demonstrates effective engagement skills; inspires
hope and promotes recovery and resiliency providing education and
community resources based on the intervention provided Demonstrates
effective documentation skills, ensures all interactions are
documented in the designated electronic medical record in
accordance with policies, procedures, and industry and/or
organizational standards of practice. Obtains signatures from
patients regarding treatment as necessary Models appropriate
physical and emotional boundaries with clients and co-workers.
Provides emotional support to help clients manage negative symptoms
and utilize coping strategies to enhance their quality of life and
daily functioning Maintains a working knowledge of the Title 36
Involuntary Commitment process completing Emergency admission and
pre-petition screening forms completely and thoroughly as
necessary. Intervenes pro-actively with clients in situations where
clients may be escalating emotionally and/or behaviorally using
approved non-violent crisis intervention techniques, including the
least restrictive interventions appropriate for the situation
Attends and participates in shift reports, site meetings, and
trainings as mandated by agency policy and procedure Communicates
effectively and promotes quality service within a compassionate
environment by treating clients and co-workers with respect and
dignity Helps to maintain a clean and orderly environment in all
client, staff and facility areas Supports a work climate that
emphasizes collective team performance and demonstrates Polara
Healths core competencies and values. Other duties as
assignedQUALIFICATIONS21 years of age or older; or minimum 18 years
old and licensed or certified under A.R.S. Title 32 and providing
services within the personnel members scope of practice; or minimum
18 years old for select administrative positionsMust be able to
provide or obtain an AZDPS Level One Fingerprint Clearance CardMust
possess and maintain a valid Driver License, current automobile
insurance, and a driving record that meets the guidelines and
requirements of the organizationCPI and CPR/First Aid certification
required (training provided by Polara Health)Master's degree in
Counseling, Psychology, Social Work, or a related field.Current
licensure or eligibility for licensure as a Licensed Professional
Counselor (LPC), Licensed Clinical Social Worker (LCSW), or similar
license in the state of Arizona.Strong ability to maintain strict
confidentiality and handle highly confidential information with
professionalismMust be able to work independently as well as
collaboratively within a team settingMust have strong interpersonal
