Job Description LanceSoft is seeking a local contract nurse RN Home Health Case Management for a local contract nursing job in Mesa, Arizona. Job Description & Requirements - Specialty: Case Management - Discipline: RN - Start Date: 07/15/2025 - Duration: 52 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Local Contract Job Title- RN Case Manager (Full-Time) Job Summary: The Case Manager (RN) provides and coordinates skilled home health services, ensuring high-quality care, patient education, and interdisciplinary collaboration. This role manages patient caseloads, oversees clinical documentation, and ensures compliance with regulatory standards. Key Responsibilities: - Conduct initial and ongoing patient assessments - Develop, implement, and update individualized Plans of Care - Provide skilled nursing services and health education - Coordinate care with physicians, team members, and external providers - Manage visit frequencies and ensure timely interventions - Prepare accurate documentation and maintain communication with physicians - Supervise home health aides and conduct bi-weekly evaluations - Identify discharge needs and facilitate transitions to advanced care - Participate in on-call rotation and weekly case conferences - Train and mentor clinical staff as needed Qualifications: - Active RN license in the state of practice - Current CPR certification - Minimum one year of home health experience - Preferred: experience with wound care, VACs, ostomy care, IV therapy - Familiarity with OASIS-E and CMS documentation standards - Basic computer skills Skills & Requirements: - Strong clinical judgment and independence - Excellent communication and interpersonal skills - Ability to manage multiple cases efficiently - Valid driver’s license, auto insurance, and background check - Negative TB test and compliance with health screenings - Field work required About LanceSoft Established in 2000, LanceSoft is a Certified MBE and Woman-Owned organization.
