Job Description Home Health Physical Therapist (PT) – Therapy Manager Titan Placement Group invites you to explore an opportunity in the charming city of Prescott, Arizona. Salary and Benefits Salary range: $108,000 to $115,000 per year Full-time, permanent position with room for advancement Health Insurance: Four plan options through Blue Cross with $300 monthly employer contribution Dental and Vision insurance offered 401(k) retirement plan available 21.5 days of paid time off per year (6.6 hours per pay period) Benefits eligibility begins the 1st of the month after 60 days Supportive environment with strong mentorship from a DPT-led administrative team Cutting-edge EMR (Synergy) with AI charting and dictation to streamline documentation Culture of communication, collaboration, and rapid responsiveness via Microsoft Teams Independent scheduling and flexible structure with oversight and assistance available when needed Responsibilities Perform approximately 20 skilled visits per week on average once ramped up Deliver quality care in patient homes while adhering to home health guidelines Complete evaluations, documentation, and follow-up visits efficiently using EMR with dictation support Operate within a 30-point productivity system (e.g., 2 points per evaluation, subject to ongoing efficiency adjustments) Collaborate with RNs, PTAs, and administrative staff to coordinate patient care Join a young and energetic therapy team committed to growth and excellence Participate in training and mentoring opportunities to develop leadership skills Eventually grow into a Therapy Manager role, overseeing therapy operations in the Prescott region Requirements Licensed Physical Therapist in the State of Arizona (active or eligible) New graduates are welcome, home health experience not required Excellent time management and communication skills Team-oriented mindset and comfort working in a dynamic, fast-paced environment Comfortable with technology, including EMR and mobile scheduling tools Eagerness to learn and grow into a leadership position About Us Titan Placement Group is a permanent placement healthcare recruiting firm that is bridging the gap between healthcare companies and high-quality candidates.
