Looking for a proactive and highly engaged Senior Care
Coordinator to join our winning team!The successful Care
Coordinator will love meeting and building relationships with
clients, conduct care consultations and find their passion in
helping clients age in their homes, have a strong focus on quality
assurance, and recognize opportunities to modify service to best

surrounding COVID-19Primary Responsibilities:
- Meet with potential clients and family members to discuss their
needs and provide solutions in the form of a service plan.
- Maintain ongoing communication and a consistent follow-up
schedule with potential clients and families who have yet to commit
to service.
- Work with other team members to coordinate various aspects of a
client's care.
- Conduct client/CAREGiver introductions with every new
client.
- Plan and execute a schedule that ensures each client has at
minimum one quality assurance visit per (month/quarter).
- Recognize and pursue opportunities to modify service plans to
best support the ongoing needs of clients through a quality
assurance program.
- Work with community senior care professionals to coordinate
service for our clients to ensure total care of client is met.
- Build relationships with these professionals to gain future
referrals.Skills:Must have knowledge of the senior-care
industryMust have 1 year experience in home care, health care or in
the senior-related industry. Must be proficient with sales force.
The successful candidate will meet/exceed monthly goals, embrace
change, will love to meet new people and build relationships. This
candidate will have excellent communication, ability to multitask,
and enjoy and promote the client/CAREGiver relationship. Must be
Organized and Forward-Thinking.Must have 1 year experience in home
care, health care or in the senior-related industry. Must be
proficient with sales force. The successful candidate will
meet/exceed monthly goals, embrace change, will love to meet new
people and build relationships. This candidate will have excellent
communication, ability to multitask, and enjoy and promote the
client/caregiver relationship. Must be Organized and
