Job Description:
A-Line Staffing is now hiring a Care Management Associate in
Phoenix, AZ ! The Care Management Associate will be working for a

details below. Care Management Associate Highlights • The pay for
this position is $18.34 per hour • Monday – Friday, 8:00 AM – 5:00
PM • Remote position, but candidates must be local to Phoenix, AZ
and willing to come onsite as needed • This position is a
full-time, contract opportunity Responsibilities • Support
coordination of medical services including screening, referrals,
and continuity of care • Create authorizations in QNXT to support
member transitions and care plans • Review incoming forms using
targeted intervention business rules and processes • Refer members
to appropriate medical services and coordinate healthcare delivery
under the guidance of clinical staff • Provide outreach to members
regarding referrals, gaps in care, and service needs • Assist with
appointment scheduling, transportation coordination, and referral
follow-up • Collaborate with case management and concurrent review
teams to facilitate appropriate outcomes • Utilize care management
tools and resources while receiving coaching and feedback for
performance improvement Requirements • High school diploma or GED
required • 1–2 years of call center experience • Strong customer
service and communication skills • Problem-solving and analytical
abilities • Proficient computer navigation and data entry skills •
Basic knowledge of medical terminology • Comfortable working in a
team-oriented and feedback-driven environment • Must be able to
pass fingerprinting requirements Benefits Available • Benefits are
